Suggestions and Guidelines for Making a Request

Please carefully review the bullets below with the three types of service requests you can choose from so that we can properly assist you:

  • Change Request - Select this option if you currently have a service and would like to modify or expand the service:
    • Add/Remove/Change ERAV group membership
    • Change VLAN on a port
    • Update Prefix List
  • New Service - Select this option if you need a service you do not currently have:
    • New WAN connectivity
    • New ERAV VPN Group
    • Managed LAN service inquiry
    • Wireless service inquiry
  • Request Support - Select this option if you require assistance with an existing service:
    • Service Outage
    • Service Degradation
    • Service Quality issues
    • Routine Maintenance

N-Wave Request Form

Please do not include sensitive information in your request.

This form allows you to request a new service, request to change an existing service or request support. The request will be routed to the appropriate group, and someone will contact you within a week to follow up. Please provide as much detail as possible about the request or issue you are observing, including any relevant URLs, data and timestamps. If you need this by a specific date/time, please indicate that here as well.
